  • Bring teller to Equitable Health Access Initiative 2B Itohan Avenue, Off Obafemi Awolowo Way, Ikeja, Lagos State OR scan your teller/Receipt of payment for an application form and mail to nestraklinikal@gmail.com ;  info@ehainigeria.org
  • Collect application form (10am-4pm) after dropping a clean and clear photocopy of the teller/or bank receipt used for payment.
  • If you shared receipt of purchase of application form via email, application form shall be mailed to your email address.
